Why does always on mean "maybe sometimes on"?

Like so many things that Samsung does, this feature just doesn't work.  Can someone please explain why my watch keeps going dark in spite of the always on setting?  It shouldn't matter, but I'm not wearing any long sleeves that are covering the watch face, nor removing it from my wrist, nor am I wearing it loosely (there is invariably an imprint in my wrist every time I remove the watch).
