Help please. When I go on device care, it doesn't clear storage anymore and I have to go through every app to clear cache.

I don't recall device care having this ability, if it did nobody would use recovery to clear system cache.

Using the HARD RESET feature of your phone (usually, switch off, then hold Bixby/Vol key/Power On), you should see a list, such as: reboot, clear cache, etc...

Right at the bottom of this list, you might see an option to REPAIR APPS - scroll down until you have highlighted this option, then press the power key to begin this. Once that's done, reboot your phone. Hopefully, you are now able to use Device Care as you were previously able to.

Power your phone off for a moment, then power up your phone in the manner described. Usually, when you see the SAMSUNG splash screen, you need to hold for another couple of seconds, until you get into the screen I described. Once you see that screen, REPAIR APPS should be at the bottom of that list.

I don't recall my s8+ ever having a (mass) clear app cache option (in device care, storage submenu), it just gave me a break down of storage allocation images/apps/music/OS reserved etc.

I used/use a Google store app called 'AppMgr III' for that functionality (automated app cache clear).
