When making a call people can't hear me but I can hear them. Video call works fine. Seems to have happened since last update.

Try running the diagnostic tool in your Samsung Members App @Members_X6ojXHV

Another troubleshooting step is  > Clear Cache Files from the phone's System Partition Section  > https://www.samsung.com/uk/support/mobile-devices/how-do-i-clear-the-cache-on-my-device/ You may need to insert your earphones or hook upto a powered On Laptop or use a Tv's USB Port to access the Clear Partition Section. This may help with lag or stutters as this clears the phone of old/unused or broken cache files.There is also a Repair Apps option in the Menu System too { Possibly excluding the s²⁴ Series of phone's }

If i can be of any further help please don't hesitate to ask  😎

The advice I offer is my own and does not represent Samsung’s position.

I have recently (belatedly) installed the latest update and I am having exactly the same issue. Normal phone calls the person on the other end hears a muffled voice, but on speaker it works fine.
There are 2 microphones - one for using the phone normally (microphone 1) and one for speaker phone calls (microphone 2). So initially I thought problem with microphone 1.
However, there's a facility on this app to test for problems. I ran it and a recording from microphone 1 is just the same as from microphone 2 - clear as a bell.
My conclusion is the OS update has introduced the fault.
I have referred the issue to the support team on the app and their experts are looking into it.
I will post an update when I have one.
