Same exact issue with mine, though mine has started after two main events.
When I got the watch I transferred my Watch 5 Pro to the 7 Ultra via Samsung Smart Switch.
All the settings and apps came across just fine.
However, over a period of two weeks, the battery had "settled in" but battery life was not performing well.
It was also reporting 1-2 days max estimation of battery life. I decided to reset the watch and to see if that would help. It was not a complete reset, meaning it was a reset from a daily back-up. This makes setting up the watch much easier.
Second Event:
I noticed that there is an app, Health Platform installed on the watch and it was draining power fast.
Reading some article online, the conclusion is the app is only used to transfer data to other workout apps.
It was also stated that Health Platform is a known battery drainer and would be replaced by another Google experiment, Health Connect. This app is not on my watch but is on my phone. Health Platform is not installed on my phone but it is on my watch! There is no way to uninstall from the watch!
I did the next best thing, and removed all permissions from the app, connections with Samsung Health, because it is not needed for this, and turned off all sensors and activity tracking. Samsung Platform no longer appears as an ongoing background app, just Samsung Health and everything works great.
Now the battery is discharging at 2.25% per hour, which is very good. I have Bluetooth, GPS turned on, continual heart reading, lots of notifications from apps, etc. WiFi, Data and AOD are off. I can toggle data on if I am away from my phone, same with GPS.
So now, the watch seems to have solid battery while still in the re-learning phase.
Samsung needs to do away with Health Platform on the watch.
All this to say, with the two events, I now have the random vibrations every so often in the exact pattern you described. 1 buzz, and then 3 buzzes later. It is maddening and I don't want to reset the watch again.
None of this happened when I first got the watch.
The third event is the recent Wear OS update to the watch earlier this week.
None of the vibrating happened before these three events.
But I see it is happening to multiple people, so it is not likely something that I did to the watch even with all the nonsense with Health Platform. Don't turn this on by the way if you have not already and connect it with Samsung Health. No need and a battery hog.

At this point, without access to detailed logs we can't pinpoint the exact cause. The online suggested solution of the vibrations being caused by Google maps mirroring did not help, but at least my watch only vibrates like less than 1 times per day on average. It's not exclusive to the watch 7 or ultra either, even my watch 4 did it occasionally, but by far way less often as compared to the new watch, like every couple of months it would do these vibrations for 2 or 3 times at most, which I can live with.
Would be great, if someone from Samsung (maybe development team?) would explain either the source or the meaning behind this vibration pattern.

My watch has stopped the random vibrations. Health Platform turned on the sensor and activity permissions that I had turned off because it seemed to be causing a battery drain. How the permissions turned themselves back on without my permission is another story, but it does seem to be the root of the issue for the random vibrations at least for my watch.
Battery life is still good.
21 hours and sleep tracking, no exercise tracking yesterday (day off) and 30% battery used. Power Savings only on overnight.
If I turn on power savings now during the day, I can get another 2 days of battery.
So, I will leave Health Platform enabled and the vibrating has stopped.

I have the same problem with my Watch 7, it is driving me crazy. I'm in contact with Samsung support since 2 weeks, no progress. I did a hard reset yesterday without restore from backup, so all from scratch and the problem is still there. Samsung is unable to solve this problem until now, no idea what will be next.
It will stop, when bluetooth is disabled, most of the time it comes back when enabling it again. However, the BT disconnect vibration is set to "none". My phone is a Samsung S22 Ultra.

Mine actually stopped with the random vibrations when Health Platform's permissions were re-enabled, by itself I might add.
I have not connected it to Samsung Health on the watch as it is not needed. Health Platform is using little to no battery. The random buzzing has stopped!

Where exactly did you deactivate this permission?
Did I understand correctly that you don't use Samsung Health on your smartphone? I'm currently using it and don't really want to do without it, or are you using something else?
Remark: Samsung has responded again and wants me to send the watch in, no further comment. I wonder if that will help?

My coworker and i both preordered the same watch and received them days apart.
Soon as i set up the watch with my phone, the same 1 and 3 vibrations started happening.
I did factory reset multiple times on it and nothing worked, including the screen mirroring from Google Maps.
I asked my coworker to set up my watch on his phone, and i set up his watch on mine.
He had my watch on for several hours, never got the vibrations, while I on the other hand was getting them on his brand new watch.
Seems there is some kind of issue between my phone and the watch, which ever it may be. Phone S24 Ultra
I sent the watch to Samsung repair, and they told me nothing was wrong and sent it right back, no issue resolved.
I am debating weather i should factory reset my phone in hopes this annoying vibrations go away

Thank you, good input. So we know, it could have something to do with Samsung Health, it is Bluetooth related and the problem has it's source probably on the phone and not on the watch. It make sense it stops when BT is disbaled, when the source is the phone.
I haven't returned the watch yet, so I ask the Samsung support what they think about a phone issue.
