Few things to check.
Check the volume controls on your headphones. Try another pair. If you're using an app for your headphones, then check the settings in that. Check your headphone manufacturer's instructions for troubleshooting.
Does the volume without headphones seem unusually low?
Settings>Sounds and Vision. Check all settings in there. Tap Volume and check all settings in there. Tap 3 dots top rh corner. Tap Media Volume limit. Check settings in there.
Settings>Accessibility> Hearing enhancements. Check all settings in there.
Boot to Safe mode
To rule out any 3rd party apps or customisations that might be causing your issues, please boot to Safe Mode.
To get to Safe Mode - Long press Power button, long press Power off icon, tap Safe mode icon. (When booted into Safe mode, you may need to turn off flight mode as it sometimes enables itself) Monitor your devices behaviour. If the issue is fixed, then you know it's one of your 3rd party apps or customisations that were the culprit. It's then up to you to determine which one by a process of elimination.

If you need to send a report, then you can also do this via your Samsung members app. Just head to the Support section and tap Error Reports.
