So, you set under do not disturb /alarms and sounds/alarms etc to be allowed and then those settings revert to off?

If you turn off the setting which turns it all on automatically and also turn off the Do Not Disturb setting this should cure the problem 👍🏻

Did you ever figure out the issue? My 22 Ultra just started doing this and I haven't found a resolution yet. I looked into what NinjaRanja said, but I've never enabled those settings so the are all off by default.

Had the same issue and here's the fix:
The phenomenon occurs due to a routine set in the phone.
Do not disturb settings changed due to this routine.
To cancel the setting, go to the device settings > Modes and routines > Tap on 'Routines' > Click on the routine you set and remove it.

For me, I didn't have routine set as @Jonathan69 mentioned.
After 2+ years, I have finally resolved this, though I'm not sure if it's one of two things, or both...
Yesterday, I upgraded from a Galaxy Watch4 Classic to a Watch7 Ultra. I decided to search this again, and came across this post, and it got me thinking. Could it be my watch? Maybe I needed to recreate my schedule, or both. Since my watch was already fresh, I deleted the schedule on my phone, rebooted, then added my schedule again. Finally, I added my wife to the allowlist, rebooted, and she was still there!
Hopefully, this will help someone out.
