As title says, the bluetooth connection to multiple devices is broken and cannot be established. It shows connecteded, but phone call audio is not going to car/headset. Also actively moving to bluetooth by selecting via button is not connecting anymore.

You are right! My so-called workaround only lasts for about a week after a reboot due to automatic optimization. I can almost confirm that the problem is caused by regular system optimization. The root cause is not the Bluetooth apps themselves, but rather one of their dependencies. I traced the dependency through Developer Options → Running Services → Bluetooth. A list of dependencies appears under Bluetooth, with the most suspicious items being SapService and Samsung Multi Connectivity. Somethings like that:
The SapService is related to the SIM card — SAP stands for SIM Access Profile — and it is not an app that can be found in the Apps menu. Anything named 'SIMxxx' is likely the target.
I have tried switching them to Unrestricted in the battery settings. So far, it has remained stable for over twelve hours. It’s very easy to test — just make a phone call to your voicemail; it’s usually free of charge.
Maybe you can try it and let me know your observations so we can work on it together.

However, I've not been about to follow the last few steps.
I can see the Bluetooth running services, including the SapService.
But the apps menu only has the SIM toolkit app. That does not have any restrictions applied (or to out it another way, I don't see any "Unrestricted" setting)
Can you advise any further? I'm desperate. I can't be turning SAP off and on again, every day.

Change your phone language to English anyway. I'm not sure that can display the same with other languages.
Via Settings, Apps, your target, battery. You should jump the options of Unrestricted; Optimized; Restricted.

Puisque cela tourne autour de l'optimisation, je viens en plus des unstricted sur les applications citées, de désinstaller les mises à jour de l'application "Maintenance du téléphone" et empêcher sa mise à jour. Comme par hasard la mise à jour de l'application date de mai 2025, où les pbs sont apparus !!
Du coup sur mon A52S, je suis en version 13.8.05.48 et la maj veut poser la version 13.8.60.4 du 5 mai 2025

Anyone tried to revert to security update before january '25 with ODIN?

This is the fix which works for me and it was hidden in Developer options:
Go to: Settings → Developer options (If you don’t see Developer options, enable it first by going to Settings → About phone → Software information and tapping Build number 7 times.)
Scroll down to the Bluetooth / Networking section.
Look for this option: “Disable A2DP hardware offload” → TURN THIS ON
Look for another option“Bluetooth SAP” → TURN THIS ON as well
After changing this, my Bluetooth calls started working properly again, no drops when switching audio to the headset. Works across all apps (normal phone calls and VoIP apps) and also in the car. No need to reboot the phone, it worked immediately after the settings were changed.
This is working for me and my wife more than 10 days already, so two A52s phones fixed with this 😊

Toggling the Bluetooth SAP has worked for me before, but only for a few hours or sometimes even a few minutes.
