If your GPS won't lock when you start an exercise, and it happens since the One UI 6 update, this reply from the awesome author of the awesome Wear GPS Fix app might help:

Thanks for the email. I do recognise the situation you describe. I think it's because the cached satellite positions are lost during the system update. A couple of suggestions. Firstly, you can try the following. Enable developer options (Google if unsure how), enable 'stay awake when charging' and attach your watch to a power supply. Place your watch as close as possible to an outside window, eg on a window sill. Start Wear GPS Fix and give it as long as it takes to get a fix (eg. leave the watch overnight). Once the fix has been obtained, the time taken subsequent fixes should be back to normal.

I have also seen people suggesting that clearing the watch's cache helps to restore normal GPS fix operation - but I haven't tried this myself.

Hope this helps.

I think also if you're just sufficiently patient and keep trying to get a fix, eventually it will work on its own, but the above is faster.

It does now. Although I've not been for a run (it's dark now) the Running app does lock GPS after the typical 13s or so, and I'm pretty sure it's going to be fine. Earlier today it hadn't locked even after five minutes. I think all you need to do is keep it in sight of the satellites for long enough, with GPS active and without the watch going to sleep, and it will eventually fix itself. The advice above is a means to keep it awake without having to sit there poking it all the time. I'm not sure if everyone will have the same experience as to how long you need to leave it. For me it was about 10-12 mins, but Malcolm (Wear GPS Fix author) was suggesting leaving it overnight. Obviously you're not going to keep it awake that long poking it by hand.

Me neither now but basically keep the watch active whilst it does the GPS lock, however long it takes. If you don't want to sit there poking the display every few seconds, there is a setting in Developer Mode to leave the screen active all the time, but you'll want the watch charging whilst using that most likely. Remember to turn the setting off after.

That said, I'm finding that this keeps happening periodically. I'll go for a run and it doesn't lock satellites. So this isn't a permanent fix.
