The app battery utilisation percentage doesn't add up to the amount of battery used, for example in the screenshot below I've used 59% and yet the total battery used in the app is less than 10% - is this normal? And if so what is is this utilisation screen??

You're measuring your battery but then using apps to calculate it.
Your watch uses battery for it's own systems, then add Bluetooth (if on) and WiFi (if on) to name but a few.
What you're looking at there is the battery usage for apps, not the device.
