My phone continuiously disconnects from my network carrier. Is this my sim card or the phone?

You can check the network coverage in your area and if there's any issues on here : https://www.ofcom.org.uk/mobile-coverage-checker

Also you mentioned that your sim card is 7-8 years old, you should ask for a new replacement sim card.
