You didn't fall hard enough. It's for falls that could cause serious injury, like falling off the top of a ladder, or down a manhole. It's not for little tumbles or stumbles, otherwise it'd be going off all the time.
I fell off my horse at a trot and while we were doing a quick 180 degree turn... the hard fall detection did not activate. Surely a fall from a horse at 10km an hour would be a hard fall?
