I had the same problem, when not connected via Bluetooth I could get and send messages and I could call out but incoming calls didn't ring in to my Watch4...yet it showed I had a missed call. The only way I can get it to receive incoming calls when not Bluetooth was to put it in stand alone mode. On wearables app on your PHONE turn OFF Remote Connection (under advanced features), then on watch under Connections turn OFF Bluetooth (and WiFi if you have it on), then under MOBILE NETWORK, change from "auto" to "always on". You are then completely in stand alone mode. Everything goes through your cellular service, so when your phone rings, the watch rings at the same time, same for incoming messages and email. Works perfectly, but drains battery faster.
