Hi @fhgf  A cuff based Monitor from the leading brands should be fine.  Obviously your Blood pressure needs to be calibrated for the Galaxy Watch 4 to take measurements and then has to be redone every 28 days.  I am using an Omron one I have  had for a while.  Some information in this video.

You can't really rely on using a watch. The best is the cuff type..do you ever see a doctor use one when you visit a surgery? I doubt you would.

Blood Pressure measurements will not be as accurate and cannot be used for medical diagnosis.   However can still be a useful guide.  This article on Galaxy watch 5 but applies to previous version too. https://www.xda-developers.com/samsung-galaxy-watch-5-blood-pressure  A study claimed features on Galaxy 4 were close to medical grade equipment,  caveats apply as was conducted at Samsung Medical Centre and paid by the company.   https://www.phonearena.com/news/Study-compares-Samsung-Galaxy-Watch-4-to-medical-grade-equipment_id1... https://www.phonearena.com/news/Study-compares-Samsung-Galaxy-Watch-4-to-medical-grade-equipment

You can buy a blood pressure monitor from Boots, that is very accurate and perfect for calibrating your Galaxy Watch 4.
I use this one for my Watch 4 Classic. They are very accurate and comparable to what a Doctor actually uses.20230322_203729_1000002177_1679517511.jpg

I should mention follow my previous post. You have to calibrate your Watch using a cuff type monitor first. Once you do that, results on the Watch are very accurate. Even my Doctor was surprised with the results in his surgery. They matched his monitor extremely accurately.
Thankfully in my case, I was only there for a checkup on my broken heel bone at the time. I don't suffer with high blood pressure.
The Watch tells you that you need to calibrate your watch with the blood pressure monitor on a fairly regular basis.

@fhgf: Further to the above, please take a look at the following link to find a step by step guide on calibrating your Galaxy Watch for use as a blood pressure monitor.

https://www.samsung.com/uk/apps/samsung-health-monitor/

If you start to the scroll down the page until you pass the 'Find a Galaxy Watch that fits your needs' section, then you will see 2 tabs - Blood pressure and ECG. Make sure that Blood pressure is selected, then continue to scroll down to find the relevant steps.
