Lately TV is erasing all the apps/icons in the home dock

Last week our Samsung TV suddenly erased all the apps and input device icons (AppleTV, PlayStation, etc) from the home dock at the bottom of the screen.  I replaced them one by one.  I did not have to actually log back into the apps, thankfully - that information was still there.  It just erased them from the home.

Tonight it happened again!  What the hell?  Why would it do that ?

That's strange @bsabiston! Could we take the full model code and software version of your TV please? Go to Home > Settings > Support > About This TV.

it also always shows a green box with “Set Up TV” when I select the TV input.  Selecting the button there does nothing.
