Yea it hasn't done it again. But then I haven't been streaming Chromecast with SoundCloud app since then.

I've figured out the culprit, it's bxActions app. Not quite sure how to replicate it but it's happend a couple of times since and I've killed bxActions which has stopped it from happening.
