Are you switching the bed mode from a watch or phone?
Did you do the usual clear cache?
I have a S22 Ultra and a Galaxy Watch 5 Pro and bed mode works as expected in both devices. Both devices synchronise with each other indistinctively on which device I activate the mode from.

To clear cache on the watch,
Slide up to open apps, select recent apps (the first one if you have not changed their orders), slide from left to right until you see close all. tap on close all and that should clear the cache on the watch.
To clear cache on the phone:
connect the phone to a computer, another phone or usb headset. Turn it off, press volume up and power simultaneously until the phone shows the Samsung logo and release the button. The phone should enter into the recovery menu. Volume up and down navigates and power selects. Navigate to Wipe Cache Partition, press power to select, use volume to navigate to 'yes' and press power to select. After clearing the cache, the phone returns to the recovery menu. You could also navigate Repair apps, press power and the phone reboots and optimise apps.
So you are turning on and off the bed mode on the watch. What happen if you disable the bed mode on the phone? The phone has a mode icon on the Quick panel, if it is not there then enter the quick panel customisation and add it.
If the bed mode is set on the watch, the mode icon turns into bed mode. when bed mode is not activated it just say mode and if activated it shows a selection of modes, bed, work, exercise, etc.
