Why when making a phone call using headphones the volume is very quiet. 

I have all volumes up to the max

It could be an issue with your headphones. If your headphones have an adjustable volume mechanism, that may need changing.

It might also be worth trying a different pair of headphones or earphones and seeing if the problem persists.

Yep I've done all this and tried 3 set of headphones. No difference

When on the samsung members page Click on the get help button and run a diagnostic test headphones

Also go settings
Volume
And check that use Volume keys for media is checked on
And you should be able to use the Volume keys

How about a soft reset
Pressing the Power key and the Volume down button for 10 seconds
All apps and data safe will just soft reset your phone and may help resolve the issue
