You could try going into settings, apps, camera, storage and clear it's cache @Moataz1
If no help then consider putting the phone in Safe Mode which disables your downloaded app's but let's your stock app's keep working. If the issues stop then this highlights a downloaded app as the possible issue. Come out of Safe Mode and start deleting your most recent downloaded app's one at a time until it stops.
Also if your using a sd card then remove it and then check just incase it's failing.

I can see people are already giving you a hand, but I'm gonna throw an extra thing into the mix. Could you go into the Samsung Members app and run a Camera Test? Could you also go into the Dialer, dial *#0*# and run the Mega Cam test in there. If those both work okay, sounds like your phone might a reset - if that fails, then you'll need a camera module replacement, as @Pugs1957 stated.
Also, before you perform a reset, make sure you back everything up, and try the suggestion that @BandOfBrothers offered.

I'm having the same problem. Have done everything suggested, two hard resets, no new apps, randomly works and then boom stops working. I'm beyond done paying over $1000 to get a decent camera to only have it stop working after a year.

Really sorry that none of our suggestions worked for you. I'm gonna suggest contacting Samsung's Customer Service to get that camera module serviced.
And honestly, I completely understand why you're annoyed - guessing the phone isn't even a year old and you're already having issues. If you're in the UK, and you've had the phone for less than 6 months, contact your retailer for a replacement unit - that'll probably be quicker than going through Sammy. Also, make sure you pull all your data off your phone.
