Mute icon when switch on TV

I have issue that when TV is switched on sometimes it shows mute icon animation on screen instead of TV channel. It can be removed only by switching between channels. I have external speakers connected and enabled instant-on. TV Reset(defaults) doesn't help.

What other devices do you have connected to it?

Only USB flash. Have not find any pattern when this shows up.
What does this icon actually mean?

What external speakers do you use and how are they connected? Does this icon appear when the speaker is connected and sound is set to play through them?

No this icon appears only when switching on TV, I have never seen it in other cases. They are regular  2.1 speakers connected to headphones 3.5mm jack. Sound is automatically played trough them when plugged in.

Does the icon appear when the speakers aren't connected?

I think not but it is not easy to repeat this issue it does happen one time in month or two. What I tried that disabling instant-on does not help.
